  • CAPÍTULO 12 - APOIO À AMAMENTAÇÃO NO PÓS-PARTO: ESTRATÉGIAS MULTIPROFISSIONAIS PARA PREVENÇÃO DO DESMAME PRECOCE

    Introdução: O aleitamento materno constitui uma prática essencial para a promoção da saúde materno-infantil, contribuindo para o adequado crescimento e desenvolvimento da criança e oferecendo benefícios nutricionais, imunológicos e relacionados ao desenvolvimento do sistema estomatognático. Apesar de suas vantagens, a manutenção da amamentação pode ser comprometida por fatores maternos, infantis, sociais, econômicos e assistenciais, favorecendo o desmame precoce. Objetivo: Analisar os principais fatores associados à manutenção do aleitamento materno, às dificuldades enfrentadas durante a amamentação e às estratégias de prevenção do desmame precoce. Metodologia: Trata-se de uma revisão integrativa da literatura, de abordagem qualitativa, realizada mediante levantamento de estudos científicos relacionados ao aleitamento materno, às dificuldades de amamentação, ao desmame precoce e às estratégias de apoio à lactação. Foram considerados estudos publicados entre 2021 e 2025, nos idiomas português e inglês, selecionados conforme sua pertinência ao tema. Resultados e discussão: Os achados evidenciaram que dificuldades como pega inadequada, sucção ineficaz, dor e fissuras mamilares, anquiloglossia, ausência de orientação e insuficiência de apoio familiar e profissional podem comprometer a continuidade do aleitamento. Fatores sociais e econômicos, como baixa renda, inserção no mercado de trabalho e baixa escolaridade, também podem favorecer o desmame precoce. Em contrapartida, ações educativas durante o pré-natal, aconselhamento individualizado, avaliação da mamada, manejo das intercorrências, acompanhamento no pós-parto, participação da rede de apoio e atuação multiprofissional demonstraram importância na manutenção da amamentação. Recursos complementares, como a fotobiomodulação para lesões mamilares, também podem auxiliar no manejo das dificuldades. Conclusão: A prevenção do desmame precoce requer assistência contínua, humanizada e qualificada, com identificação precoce das dificuldades e fortalecimento das ações de promoção, proteção e apoio ao aleitamento materno em todos os níveis de atenção à saúde.

ABSTRACT:
Introduction: Breastfeeding is an essential practice for promoting maternal and child health, contributing to the child' s adequate growth and development while offering nutritional, immunological, and stomatognathic development benefits. Despite its advantages, the continuation of breastfeeding can be compromised by maternal, infant, social, economic, and healthcare-related factors, leading to early weaning. Objective: To analyze the main factors associated with the continuation of breastfeeding, the difficulties encountered during the process, and strategies to prevent early weaning. Methodology: This is an integrative literature review with a qualitative approach, conducted through a survey of scientific studies related to breastfeeding, breastfeeding difficulties, early weaning, and lactation support strategies. Studies published between 2021 and 2025 in Portuguese and English were considered, selected based on their relevance to the topic. Results and discussion: Findings showed that difficulties such as improper latch, ineffective sucking, pain and nipple fissures, ankyloglossia, lack of guidance, and insufficient family and professional support can compromise the continuation of breastfeeding. Social and economic factors—such as low income, workforce participation, and low levels of education—can also contribute to early weaning. Conversely, educational actions during prenatal care, individualized counseling, breastfeeding assessment, management of complications, postpartum follow-up, support network involvement, and multidisciplinary care proved important for maintaining breastfeeding. Complementary resources, such as photobiomodulation for nipple lesions, can also assist in managing difficulties. Conclusion: Preventing early weaning requires continuous, humanized, and high-quality care, involving the early identification of difficulties and the strengthening of actions to promote, protect, and support breastfeeding across all levels of healthcare.
