  • CAPÍTULO 13 - ATENÇÃO À SAÚDE DE PESSOAS PRIVADAS DE LIBERDADE: DESAFIOS E ESTRATÉGIAS PARA A INTEGRALIDADE DO CUIDADO

    Introdução: A saúde é um direito fundamental garantido pelo Estado à todos, inclusive à população privada de liberdade. Entretanto, a grave crise estrutural dos presídios desafia a efetivação dessa assistência integral. Objetivo: Analisar os principais desafios enfrentados e as estratégias implementadas para a garantia da integralidade do cuidado na atenção à saúde das pessoas privadas de liberdade. Metodologia: Trata-se de uma revisão integrativa da literatura, realizada por meio das bases de dados MEDLINE e LILACS. A estratégia de busca foi estruturada a partir dos Descritores em Ciências da Saúde. Foram incluídos estudos primários e secundários, publicados entre 2021 e 2026, em português ou inglês, disponíveis em texto completo e com pertinência direta à questão norteadora. Foram excluídas teses, dissertações, monografias, editoriais, comentários e literatura não convencional. Ao final, 10 estudos foram selecionados para compor a análise. Resultados e Discussão: A análise demonstra que a efetivação da saúde no sistema prisional brasileiro esbarra em barreiras estruturais, superlotação e na subordinação da assistência às normas de segurança. Embora a legislação garanta o direito à atenção integral e as políticas públicas busquem expandir as ações do Sistema Único de Saúde para o cárcere, a fragmentação do cuidado ainda predomina nas unidades. Diante desse cenário de negligência e estigma social, a abordagem multiprofissional e o uso da telemedicina destacam-se como estratégias essenciais. Considerações Finais: Conclui-se que há uma grande distância entre as leis e a realidade da saúde prisional, onde o cuidado é fragmentado e limitado por superlotação e falta de recursos. Para garantir a integralidade, não bastam apenas normas, mas investimentos estruturais, equipes multiprofissionais e articulação intersetorial.

ABSTRACT:
Introduction: Health is a fundamental right guaranteed by the State to everyone, including the incarcerated population. However, the severe structural crisis in prisons challenges the effective delivery of this comprehensive care. Objective: To analyze the main challenges faced and the strategies implemented to ensure comprehensive healthcare for incarcerated individuals. Methodology: This is an integrative literature review conducted using the MEDLINE and LILACS databases. The search strategy was structured based on Health Sciences Descriptors (DeCS). Primary and secondary studies published between 2021 and 2026—in Portuguese or English, available in full text, and directly relevant to the guiding question—were included. Theses, dissertations, monographs, editorials, commentaries, and non-conventional literature were excluded. Ultimately, 10 studies were selected for the analysis. Results and Discussion: The analysis demonstrates that the implementation of healthcare within the Brazilian prison system faces structural barriers, overcrowding, and the subordination of care to security protocols. Although legislation guarantees the right to comprehensive care and public policies aim to extend the Unified Health System' s services to the prison environment, fragmented care remains prevalent in these facilities. Given this context of neglect and social stigma, a multidisciplinary approach and the use of telemedicine stand out as essential strategies. Final Considerations: It is concluded that there is a significant gap between the laws and the reality of prison healthcare, where care is fragmented and constrained by overcrowding and a lack of resources. Ensuring comprehensive care requires not only regulations but also structural investments, multidisciplinary teams, and intersectoral coordination.
