ABSTRACT:
Introduction: Gestational diabetes mellitus (GDM) is a common metabolic disorder during pregnancy associated with maternal, fetal, and neonatal consequences, making screening and glycemic control important components of prenatal care. Objective: To analyze aspects related to GDM screening, management, and maternal-fetal outcomes, emphasizing the strategies used for its control during pregnancy. Methodology: This is an integrative literature review with a qualitative approach and descriptive nature, conducted using the PubMed, SciELO, and LILACS databases. It considered full-text publications from the last five years in Portuguese, English, and Spanish. Results and Discussion: GDM diagnosis involves prenatal screening, notably the 75g oral glucose tolerance test, usually performed between the 24th and 28th weeks of gestation. Management includes individualized nutritional therapy, physical activity, and blood glucose monitoring; insulin therapy is indicated when non-pharmacological measures fail to achieve established targets. Metformin may be used in specific situations, although guidelines differ regarding its safety and potential effects on offspring. Adequate control of hyperglycemia is associated with a reduction in maternal and neonatal complications, whereas a history of GDM increases the risk of recurrence in future pregnancies and the development of type 2 diabetes mellitus. Final Considerations: Timely screening, individualized monitoring, and the integration of nutritional, physical, pharmacological, and educational measures are fundamental for controlling GDM and reducing maternal-fetal consequences, with postpartum follow-up also being necessary.
